Applying for Assistance
Apply for services such as:
- Medical Assistance
- Food & Nutrition Services (FNS)
- Energy Assistance (LIEAP/CIP)
There are three ways to apply for Food and Nutrition Services (Food Stamps):
1. Online
Upon completion of the online application, mail your form to:

2. In Person
You will need to bring the following items with you when you apply:
- ID for person applying
- Social Security numbers for all household members
- All check stubs received in the month prior to application for every household member that is employed
- Self-employment income needs to be verified by the prior year’s income tax statement
- Verification of any other income coming into the home such as but not limited to child support, contributions, pensions, etc.
- Current rent receipt or mortgage statement (if mortgage, provide property tax statement and homeowner’s insurance if separate from mortgage)
3. By Mail
We need your name, address and phone number if we need to call you for clarification purposes.
Application packets are prepared to be mailed daily.

Lost/Stolen EBT card
- Lost
If you have a lost EBT card, call (888) 622-7328 and follow the prompts.
- Stolen
If your card has been stolen, please take the following steps:
Call (888) 622-7328 so your card can be deactivated.
Call the Sheriff’s department at (336) 634-3232 to report the theft.
