Certified Copy Requirements for Vital Records
Certified copies of vital records are limited to immediate family members or authorized agents and require valid photo identification. See below for a list of authorized individuals and acceptable forms of identification.
Individuals authorized to obtain certified copies of vital records:
- Yourself
- Your spouse
- Sibling (brother, sister, step-brother, step-sister)
- Child or step-child
- Grandchild
- Grandparent
- Individuals seeking information for legal determination of personal or property rights
- An authorized agent, attorney or legal representative of the person whose certificate is being requested
Acceptable forms of identification
- Valid Driver's License or Identification Card issued by state or federal government
- Valid Passport
- Valid Armed Forces ID

Adoption Records
Birth records of individuals who were adopted are not available in our office. Please contact NC Vital Records at (919) 733-3526 for assistance with adoption records.
