Information Needed to Request a copy of an Environmental Health Historical Record

The following form must be completed fully whenever requesting an Environmental Health historical file for your property or property that you are considering for:

• Septic System Maintenance
• Real Estate Listing or Transaction
• Home Replacement
• Additions or replacement of a pool, deck, room(s) onto an existing home, outbuilding or garage whether attached or not
• Appraisal
• A recent purchase
• Landscaping or grading
• General information and possible future use/need

Incomplete or missing information will delay the delivery of your request. Sometimes Environmental Health historical files are in the name of a contractor. Mobile and modular homes have also been listed in the name of a mobile home company (Ex. Clayton Homes, First Choice Housing, Oakwood of Eden, etc.)

If you do not know, ask a neighbor that might possibly know who built the home if you are not the original owner.
